An artificial spinal implant is disclosed which when placed between two adjacent vertebrae directly participates and is incorporated in the ensuing fusion. Instrumentation and procedure is also disclosed. The implant of the present invention consists of a generally cylindrical member having a series of openings and threads on its exterior surface. A guard and drill guide in the form of a hollow sleeve is provided that is driven into the vertebrae so as to maintain the vertebrae fixed relative to one another. An opening is drilled in the vertebrae. A thread is tapped in the opening made by the drill. The threaded implant is then inserted into the opening and screwed in the vertebrae, with the sleeve being maintained in place during the entire procedure. After the entire procedure is completed the sleeve is then removed.
