This invention is embolectomy catheters (11), rapid exchange micro-catheters (265), systems, and methods for removing obstructive matter (e.g., thrombus, thromboemboli, embolic fragments of atherosclerotic plaque, forein objects, etc.) from blood vessels. This system is particularly useable for percutaneous removal of thromboemboli or other obstructive matter from small blood vessels of the brain, during an evolving stroke or period of cerebral ischemia. In some embodiments, the embolectomy catheters are advanced over a guide wire (GW) which has been pre-inserted through or around the obstructive matter. These catheters include obstructive matter capturing receptacles (14) which are deployed after the catheter has been advanced at least partially through the obstructive matter. The receptacles may include at least one proximal strut (20), which is designed to be retractable through a blood clot and a distal matter-receiving portion (16) which is designed to prevent a blood clot from passing therethrough.
L'invention porte sur des cathéters d'embolectomie, des microcathéters à échange rapide, des systèmes et des procédés d'élimination de matières obstructives (telles que thrombus, thrombo-embolies, fragments emboliques de la plaque d'athérosclérose, objets étrangers, etc.) des vaisseaux sanguins. Cette invention peut également s'appliquer à l'élimination par voie percutanée de thrombo-embolies ou autres matières obstructives des petits vaisseaux sanguins du cerveau lors d'une attaque cérébrale évolutive ou d'une ischémie cérébrale. Dans certaines réalisations, il est possible de faire avancer les cathéters d'embolectomie de cette invention sur un fil de guidage qui a été pré-inséré dans les matières obstructives ou autour de celles-ci. Dans d'autres réalisations, ces cathéters d'embolectomie comportent des réceptacles de capture des matières obstructives qui font saillie du cathéter dès qu'on a fait progresser celui-ci au moins partiellement dans les matières obstructives. Les réceptacles de capture des matières obstructives peuvent comporter i) au moins un élément de renforcement proximal conçu pour se rétracter dans un caillot de sang et ii) une partie distale accueillant les matières et conçue pour empêcher le passage d'un caillot de sang.
