The present invention provides stabilizing devices and methods of use for temporarily remotely immobilizing a local area of tissue, such as a local area of tissue on a beating heart, or other internal organ, to thereby permit minimally-invasive or robotic surgery thereon. The invention stabilizing device, which is coupled to a vacuum source, features an elastomeric suction body mounted on the steerable tip of a rod-like instrument, such as a steerable catheter. The flexible or steerable tip and elongate suction channel are sized for introduction into an interior body cavity via a small surgical opening. A steering mechanism mounted at the proximal end of the device is used to coil the elongate tip and suction channel into a ring- or horseshoe- shape within the body cavity. Preferably, the suction channel has a flexible, spreadable outer rim portion which is designed to flatten and spread outwardly against the surface of the treatment site as air is withdrawn from the coiled suction channel to distribute the pressure against the treatment site while maximizing the suction field. The suction body is manufactured of a soft, compliant material, such as an elastomeric polymer, to allow the suction channel to be transformed into the coiled conformation without deformation sufficient to defeat its purpose of use, i.e., clinging to the surface of a treatment site under partial vacuum.
La présente invention porte sur des dispositifs de stabilisation et des procédés d'utilisation visant à immobiliser provisoirement à distance une zone locale d'un tissu, par exemple, une zone sur le coeur en mouvement, ou sur un autre organe interne, de façon à pouvoir réaliser une chirurgie effractive minimale ou robotique. Ce dispositif de stabilisation, couplé à une source de vide, se caractérise par un corps d'aspiration élastomère monté sur l'embout orientable d'un instrument de type tige tel qu'un cathéter orientable. L'embout flexible ou orientable et le canal d'aspiration allongé sont dimensionnés de façon à pouvoir être introduits dans une cavité interne du corps par un petit orifice chirurgical. Un mécanisme de direction monté sur l'extrémité proximale du dispositif est utilisé pour enrouler l'embout allongé et le canal d'aspiration sous forme d'un anneau ou d'un fer à cheval dans la cavité du corps. De préférence, le canal d'aspiration comporte une partie marginale externe, extensible, flexible, conçue pour s'aplatir et s'étaler vers l'extérieur contre la surface du site de traitement lorsque l'air est retiré du canal d'aspiration enroulé de façon à répartir la pression sur le site de traitement tout en maximisant le champ d'aspiration. Le corps d'aspiration est fabriqué dans un matériau souple tel qu'un polymère élastomère de façon à pouvoir transformer le canal d'aspiration en une configuration enroulée sans qu'il subisse une déformation qui le rendrait inopérant, c'est-à-dire en adhérant à la surface d'un site de traitement sous vide partiel.
