An implantable medical interventional device (10) is responsive to detection of any of a plurality of cardiac dysrhythmias in a human patient by performing an appropriate therapy including cardiac pacing, cardioversion or defibrillation according to the nature of the detected dysrhythmia. The device includes a first sensor means (13, 21) for detecting any of the plurality of cardiac dysrhythmias, and a generator (15, 20) for developing pulses and shocks for delivery to the patient's heart according to whether a detected dysrhythmia is bradycardia or a relatively slow pathologic tachycardia on the one hand, or a relatively fast tachycardia or fibrillation on the other hand. An optimizer (22, 24) in the device seeks to maintain at all times a substantial match of the patient's heart rate to the normal rate for a healthy person under like conditions of physical exercise, including relatively minor activity, and rest experienced by the patient. The optimizer includes a second sensor (30) to detect when the patient is engaged in physical exercise or rest as imposing different hemodynamic demands on the patient's cardiovascular system, and to produce a signal representative of the then-current hemodynamic demand. A signal processor (31) of the device enhances the signal to distinguish components thereof representing true physical exercise by the patient and the extent of such exercise from components of the signal constituting false indications of activity by the patient. The enhanced signal is applied to the generator (15) to develop pulses for delivery to the patient's heart to vary the heart rate to conform to the then-current hemodynamic demand on the patient attributable to the conditions of physical exercise or rest. In that way, the device focuses on correction of cardiac pacing problems before they become sufficiently aggravated to require more aggressive cardioversion and defibrillation therapies.
Un dispositif médical interventionnel implantable (10) répond à la détection de n'importe quelle dysrythmie d'une pluralité de dyrhytmies cardiaques chez un patient humain en appliquant une thérapie appropriée, notamment une stimulation cardiaque, une cardioversion ou une défibrillation selon la nature de la dysrythmie détectée. Le dispositif comprend un premier moyen capteur (13, 21) destiné à détecter n'importe quelle dysrythmie de la pluralité de dysrythmies cardiaques, et un générateur (15, 20) destiné à développer des impulsions et des chocs à administrer au coeur du patient selon que la dysrythmie détectée est une bradycardie ou une tachycardie pathologique relativement lente d'une part, ou bien une tachycardie ou fibrillation relativement rapide d'autre part. Un optimiseur (22, 24) du dispositif cherche à maintenir à tout moment une correspondance substantielle entre la fréquence cardiaque du patient et la fréquence normale d'une personne en bonne santé dans des conditions similaires d'exercice physique, notamment une activité relativement mineure, et le repos du patient. L'optimiseur comprend un second capteur (30) destiné à détecter à quel moment le patient est en exercice physique ou au repos, ce qui impose différentes conditions hémodynamiques au système cardiovasculaire du patient, et afin de produire un signal représentatif de la condition hémodynamique alors courante. Un processeur (31) de signal du dispositif améliore le signal afin d'en distinguer les composantes représentant l'exercice physique vrai du patient et le degré de cet exercice à partir des composantes du signal constituant des indications fausses d'activité du patient. Le signal amélioré est appliqué au générateur (15) afin de développer des impulsions à administrer au coeur du patient, afin de varier la fréquence cardiaque pour qu'elle se conforme à la condition hémodynamique alors courante du patient se rapportant aux conditions d'exercice physique ou de repos. Ainsi, le dispositif se concentre sur la correction des problèmes de stimulation cardiaque avant qu'ils ne deviennent suffisamment aggravés pour exiger des thérapies de cardioversion et de défibrillation plus agressives.
