The present invention relates to the use of at least one Killer cell immunoglobulin-like receptor gene for the identification of subjects likely to respond to tumour necrosis factor-based therapy. The invention also provides methods of identifying subjects likely to respond to tumour necrosis factor-based therapy, and finds utility in assisting with prospectively predicting the likely clinical response of patients to therapeutic agents used in TNF therapy, based on the genotype of the patient, in particular, by evaluating the status of at least one KIR gene. This predictive tool finds wide clinical utility in the management of autoimmune diseases, such as rheumatoid arthritis, and greatly assists clinicians in the prioritisation of patients for tumour necrosis factor- based therapy, possibly reducing the cost of treatment in terms of adverse side effects and health budgets.

L'invention porte sur l'utilisation d'au moins un gène récepteur de cellules tueuses, du genre immunoglobuline, pour l'identification de patients susceptibles de répondre à une thérapie à base de facteur de nécrose tumorale (TNF), et sur les méthodes d'identification associées. L'invention constitue une aide à la prédiction prospective de la probabilité de réponse clinique de patients aux agents thérapeutiques utilisés en thérapie par le TNF et se basant sur le génotype du patient, en particulier en évaluant l'état d'au moins un gène KIR. Cet outil prédictif présente une large utilité clinique dans la gestion de maladies auto-immunes telles que la polyarthrite rhumatoïde et aide énormément les cliniciens dans la sélection de patients susceptibles de recevoir une thérapie basée sur le TNF, sélection pouvant réduire le coût du traitement en termes d'effets secondaires défavorables et de budget de santé.
