A wastewater treatment apparatus (5, 200) includes a continuously rotating drum-type filter screen (10, 210) having a central chamber (18, 220). Wastewater flows into the chamber (18, 220) and out through the bottom (22, 224) and sides (24, 226) of the screen (10, 210). Large-sized solids are captured by the screen (10, 210), then dislodged from the top of the screen (10, 210). Dislodged solids are directed into a grinder unit (100, 240) that macerates the solids.
