Surgical techniques and instruments are provided for performing surgical procedures in a disc space. The instrument (45) includes a sleeve (50) having a distal end (56) and an opposite proximal end (54) and a working channel (52) extending therebetween. The working channel (52) includes a first portion (52a) and an adjacent second portion (52b) configured to receive surgical instruments therethrough. A cap member (150) is secured to the sleeve (50) at the proximal end (54). The cap member (150) has a sealable access port (154) substantially aligned with a corresponding one of the first or second portions (52a, 52b). The cap member (150) is movable with respect to the sleeve (50) to substantially align the access port (154) with the other of the first and second portions (52a, 52b) while the working channel (52) remains sealed. Various techniques using various surgical instruments in laparoscopic procedures are also disclosed.
