An oral appliance (10) for repositioning the temporomandibular joint having a base portion (11) and inner and outer flanges (13 and 12) which form upper and lower U-shaped channels (15 and 16) for receiving the teeth of the upper and lower jaws. The inner flange (13) includes a central tab (27) and a pair of slots (28) on opposite sides of the tab (27) to allow for lateral adjustment of the appliance. In another embodiment, the oral appliance (10) has a base portion (11), inner and outer flange portions (13, 12) extending to opposite sides of the base portion (11) to form upper (15) and lower (16) channels. The outer flange portion (12) has rib means (44) on its inner surface on opposite sides of and spaced from the base portion (11) extending parallel to the base portion (11) to engage the teeth of the user.
