A method is disclosed for introducing a spinal disc implant into an intervertebral space of a subject. The subject is placed in a lateral position, and the anterior face of the spinal disc intervertebral space is accessed, between the L5 and S1 vertebrae, from an anterior and lateral retroperitoneal approach. An operative corridor to the anterior face of the spinal disc space is established by introducing a retractor instrument anterolaterally to the spinal disc space between the anterior superior iliac spine and the anterior inferior iliac spine. The damaged spinal disc contents are removed from the intervertebral space through the operative corridor, and the implant is advanced into the intervertebral space at an oblique angle and pivoted to position the implant substantially laterally within the intervertebral space. Elongated retractor and insertion instruments, as well as a modified disc implant, are also disclosed for carrying out the method.
L'invention concerne un procédé permettant d'introduire un implant de disque rachidien dans un espace intervertébral chez un sujet. On place le sujet en position latérale et l'on accède à la face antérieure de l'espace intervertébral du disque rachidien, entre les vertèbres L5 et S1, avec une approche rétropéritonéale latérale et antérieure. On établit un couloir fonctionnel vers la face antérieure de l'espace de disque rachidien en introduisant un instrument rétracteur de manière antérolatérale par rapport à l'espace de disque rachidien entre l'épine iliaque supérieure antérieure et l'épine iliaque inférieure antérieure. On retire les contenus de disque rachidien endommagés de l'espace intervertébral à travers le couloir fonctionnel et l'on fait avancer l'implant dans l'espace intervertébral à un angle oblique et on le fait pivoter pour disposer l'implant sensiblement latéralement dans l'espace intervertébral. L'invention concerne également un rétracteur allongé et des instruments d'insertion de même qu'un implant pour disque modifié, pour réaliser le procédé.
