In vitro diagnosis/prognosis method and kit, for assessment of tolerance in liver transplantation. The present invention refers to the study of peripheral blood transcriptional patterns from 80 liver transplant recipients and 16 non-transplanted healthy individuals employing either oligonucleotide microarrays and/or quantitative real-time PCR to design a clinically applicable molecular test. This has resulted in the discovery and validation of several gene signatures comprising a modest number of genes capable of identifying tolerant and non- tolerant recipients with high accuracy. The marker genes are KLRF1, SLAMF7, NKG7, IL2RB, KLRB1, FANCG, GNPTAB, CLIC3, PSMD14, ALG8, CX3CR1, RGS 3. Multiple peripheral blood lymphocyte subsets contribute to the tolerance-associated transcriptional patterns with NK and .gamma.delta T cells exerting a predominant influence. The invention concludes that transcriptional profiling of peripheral blood can be employed to identify liver transplant recipients who can discontinue immunosuppressive therapy and that innate immune cells are likely to play a major role in the maintenance of operational tolerance in liver transplantation.

La présente invention concerne un procédé de diagnostic/pronostic in vitro et un kit, pour évaluer la tolérance de la transplantation du foie. La présente invention se rapporte à l'étude de motifs de la transcription du sang périphérique provenant de 80 bénéficiaires dune transplantation du foie et de 16 individus sains non transplantés utilisant des matrices oligonucléotidiques et/ou une PCR quantitative en temps réel pour concevoir un test moléculaire applicable cliniquement. Ceci a résulté en la découverte et la validation de plusieurs signatures génétiques comprenant un petit nombre de gènes capables d'identifier des bénéficiaires tolérants et non tolérants avec une grande précision. Les marqueurs génétiques sont les KLRF1, SLAMF7, NKG7, IL2RB, KLRB1, FANCG, GNPTAB, CLIC3, PSMD14, ALG8, CX3CR1, RGS 3. De multiples sous ensembles de lymphocytes du sang périphérique contribuent à des motifs de la transcription associés à la tolérance avec des lymphocytes T NK et ?delta exerçant une influence prédominante. L'invention conclut que le profilage de la transcription du sang périphérique peut être utilisé pour identifier des bénéficiaires de transplantation du foie qui peuvent arrêter la thérapie immunosuppressive et que les cellules immunitaires innées sont susceptibles de jouer un rôle majeur dans le maintien d'une tolérance opérationnelle de la transplantation du foie.
