The transendothelial migration of leukocytes and other cells during acute and/or chronic inflammation can be inhibited by the administration of anti-PECAM-1 reagents. In particular, transendothelial migration appears to be mediated by the activity of platelet endothelial cell adhesion molecule (PECAM); as inhibition of neutrophil and monocyte TEM occurs in the presence of recombinant soluble PECAM or a monoclonal anti-PECAM antibody. Methods for controlling transendothelial migration and corresponding agents are disclosed, as arc related diagnostic methods and drug discovery assays. Inhibition of such transendothelial migration will play a role in the prevention and/or diminishment of organ damage in inflammation, and the present method may be especially useful in the treatment of chronic inflammation, as well as in treating acute inflammation.
