Devices and methods are provided for the ablation of regions of the digestive tract to achieve hemostasis and to eradicate chronically bleeding lesions as occur with gastric antral vascular ectasia (GAVE), portal hypertensive gastropathy (PHG), radiation proctopathy and colopathy, arteriovenous malformations, and angiodysplasia. Ablation is typically provided in a wide-field manner, and in conjunction with sufficient pressure to achieve coaptive coagulation. Ablation, as provided the invention, starts at the mucosa and penetrates deeper into the gastrointestinal wall in a controlled manner. Ablation control may be exerted by way of electrode design and size, energy density, power density, number of applications, pattern of applications, and pressure. Control may also be provided by a fractional ablation that ablates some tissue within a target region and leaves a portion substantially unaffected. Embodiments of the device include an ablational electrode array that spans 360 degrees and an array that spans an arc of less than 360 degrees.
La présente invention concerne des dispositifs et des procédés permettant de réaliser l'ablation de régions du tube digestif afin d'obtenir une hémostase et supprimer les lésions à saignement chronique qui apparaissent, par exemple, lors d'une ectasie vasculaire antrale (GAVE), gastropathie portale hypertensive (PHG), proctopathie et colopathie irradiantes, malformations artérioveineuses et angiodysplasie. L'ablation est effectuée généralement avec un grand champs visuel et avec suffisamment de pression pour pouvoir parvenir à une coagulation coaptive. Selon l'invention, l'ablation commence au niveau de la muqueuse et pénètre plus profondément dans la paroi gastro-intestinale de manière contrôlée. La conception et la taille de l'électrode, la densité de l'énergie, la densité de la puissance, le nombre d'applications, le motif d'applications et la pression peuvent permettre de contrôler l'ablation. Il est également possible d'exercer un contrôle au moyen d'une ablation fractionnelle effectuée sur certains tissus dans une région cible et qui laisse une partie sensiblement intacte. Les modes de réalisation du dispositif comprennent une grille d'électrodes d'ablation qui couvre 360 degrés et une grille qui couvre un arc inférieur à 360 degrés.
