An apparatus is disclosed for guiding a medical instrument to a predetermined target location within the periurethral tissues of a patient. The apparatus directs the instrument along a predetermined path with respect to the patient's urethra (250) and limits penetration of the instrument to a predetermined depth. The disclosed embodiment comprises an apparatus for implanting inflatable prostheses (220) within the periurethral tissues to coapt the urethra (250) and thereby manage incontinence. A method for effecting coaptation of a urethra (250) of a patient is also disclosed. According to this method, a pair of working channels are formed within the periurethral tissues, and an inflatable prosthesis (220) is introduced into each working channel. The prostheses (220) are inflated only after both working channels have been formed, and preferably after both prostheses (220) have been positioned within their respective working channels. The inflated prostheses (220) confront the urethra (250) and effect coaptation of the urethra (250).
Appareil servant à guider un instrument médical vers un endroit cible prédéterminé situé à l'intérieur des tissus périurétraux (250) d'un patient. Cet appareil dirige l'instrument le long d'un trajet prédéterminé par rapport à l'urètre du patient et limite la pénétration de l'instrument à une profondeur prédéterminée. La forme de réalisation décrite concerne un appareil permettant d'implanter des prothèses gonflables (220) à l'intérieur des tissus périurétraux pour effectuer la coaptation de l'urètre (250) et ainsi pallier l'incontinence. Cette invention concerne également un procédé permettant d'effectuer la coaptation de l'urètre d'un patient, selon lequel on forme deux canaux fonctionnels dans les tissue périurétraux puis on introduit dans chacun d'eux une prothèse gonflable (220). On ne gonfle les prothèses (220) qu'après avoir formé les deux canaux fonctionnels et de préférence qu'après avoir positionné les deux prothèses (220) à l'intérieur de leur canal respectif. Les prothèses gonflées (220) bloquent l'urètre (250) et produisent sa coaptation.
