An apparatus and method of intraoperative myocardial revascularization of the myocardium of the heart of a patient. A catheter apparatus comprising an elongated catheter, an elongated laser wave guide slidably disposed within a lumen of the catheter, and an ultrasonic transducer secured to the distal end of the elongated laser wave guide, is inserted into the patient. The distal end of the leasing apparatus is guided to the portion of the patient's heart wall in which channels will be formed, and the ultrasonic transducer is activated to create brief pulses of ultrasonic energy. The transducer receives a returned ultrasonic echo from the heart wall. The ultrasonic echo is processed by signal processing elements. The processed ultrasonic echoes are displayed to show the distance between the epicardial and endocardial surfaces of the portion of the heart wall in which the revascularization energy is to be discharged, and the distance between the operative distal end of the myocardial revascularization device and such endocardial and epicardial surfaces. After distance measurements have been performed, channels are formed in the heart wall.
Dispositif et procédé de revacularisation myocardique péropératoire du myocarde du coeur d'un patient. Un dispositif cathéter comporte un cathéter allongé, un guide d'onde laser allongé disposé coulissant à l'intérieur d'une lumière du cathéter, et un transducteur à ultrasons fixé à l'extrémité distale du guide d'onde laser allongé. Ledit dispositif est introduit dans le corps du patient. L'extrémité distale de l'élément à effet laser est guidée vers la partie de la paroi du coeur du patient dans laquelle des vaisseaux seront formés. Le transducteur à ultrasons est activé en vue de créer de brèves impulsions d'énergie ultrasonore. Le transducteur reçoit un écho ultrasonore renvoyé à partir de la paroi du coeur. Ledit écho est traité par des éléments de traitement de signaux. Les échos ultrasonores traités sont affichés en vue de présenter la distance existant entre les surfaces épicardiques et endocardiques de la partie de la paroi du coeur dans laquelle l'énergie de revascularisation doit être déchargée, et la distance existant entre l'extrémité distale opératoire du dispositif de revascularisation myocardique et lesdites surfaces. Après que les mesures de distances aient été effectuées, des vaisseaux sont formés dans la paroi du coeur.
