The present invention is an apparatus for detecting fault conditions, such as missing or improper hardware, in the operation of a fastener press (10). It includes a workpiece die (26) that is aligned and cooperates with a punch assembly (18) in the insertion of the fasteners (40) into holes in the workpiece (38). The die has a linear displacement member (44) mounted for reciprocal movement in response to the operation of the punch assembly (18). The position of the linear displacement member (44), which represents whether or not a fault condition exists, is detected and an alarm indication is generated when there is a fault.
