Apparatus and methods for treating in-stent restenosis are described for removing stenotic material from within previously stented regions of a patient's vasculature. The apparatus includes a catheter system having a stenotic material removal mechanism (52) mounted on a distal portion of an elongated inner catheter (50). A sensing means (54), such as one or more sensing electrodes, are positioned on an outer surface of the apparatus. In addition, the apparatus optionally includes control means for diametrically expanding the stenotic material removal mechanism for effective recanalization of the stent. A coaxial outer catheter (60) is provided for aspirating stenotic material which is removed from within the stent. In additions embolic filter apparatus (90) are described for collecting the stenotic material removed from within the stent.

L'invention concerne un appareil et des procédés permettant de traiter une resténose dans une prothèse endovasculaire, par extraction de la matière sténosée des régions du système vasculaire d'un patient préalablement dotées d'une prothèse endovasculaire. L'appareil comprend un système de cathéter pourvu d'un mécanisme d'extraction (52) de matière sténosée, monté sur une partie distale d'un cathéter (50) intérieur allongé. On place un organe de détection (54) , tel qu'une ou plusieurs électrodes de détection, sur une surface extérieure de l'appareil. De plus, l'appareil comprend, éventuellement, un système de commande permettant d'élargir diamétralement le mécanisme d'extraction de la matière sténosée, afin d'obtenir une recanalisation efficace de la prothèse endovasculaire. Un cathéter coaxial extérieur (60) sert à aspirer la matière sténosée extraite de la prothèse endovasculaire. L'invention concerne également un appareil filtrant anti-embolie (90) destiné à collecter la matière sténosée extraite de la prothèse endovasculaire.
