A method is provided for forming an autologous graft by distending a donor blood vessel, and harvesting the distended portion of the vessel. Also provided is a device for in vivo, or in vitro vessel distension. Typically the devise is implanted, for example, using endoscopic techniques, in a patient sometime prior to another surgery on that patient, which includes implanting a vascular graft. The device includes a stretching mechanism which is attached to a donor blood vessel, means for operating the stretching mechanism to cause the vessel to distend, and a controller, preferably externally located, for controlling the operating means. In a preferred embodiment, the device includes a pair of opposed straps (10, 12) attached to a healthy, small diameter donor blood vessel such as a femoral artery in the leg. The straps (10, 12) are displaced from each other over a period of time to continuously distend or elongate the donor vessel. The distended portion of the donor vessel is excised at the time of the bypass surgery. The ends of the donor vessel are then sutured end to end to repair the donor vessel, a procedure common in vascular repair, and generally accomplished without complication. The result is a totally autologous, living vascular graft.

L'invention porte sur un procédé de formation d'une greffe autologue consistant à distendre un vaisseau sanguin donneur puis à recueillir la partie distendue, et sur un dispositif permettant la distension in vivo ou in vitro de vaisseaux. Normalement, on implante le dispositif chez le patient, par exemple à l'aide de techniques endoscopiques, peu avant une autre opération comportant l'implantation d'une greffe vasculaire. Le dispositif comporte un mécanisme distendeur se fixant au vaisseau donneur, et une commande de préférence extérieure dudit mécanisme. Dans une exécution préférée, le dispositif comporte une paire de sangles opposées (10, 12) fixées à un vaisseau sanguin sain donneur de petit diamètre tel que l'artère fémorale de la jambe, lesquelles sangles (10, 12) sont écartées l'une de l'autre pendant le temps nécessaire pour les distendre et de les allonger, puis la partie distendue est excisée au moment de la réalisation du pontage. On suture ensuite bout à bout les extrémités du vaisseau donneur pour le refermer, ce qui est une opération courante généralement sans complications. On réalise ainsi une greffe vasculaire vivante totalement autologue.
