A barrel accessory rail system (10) includes a pair of cooperating clamping sections (55a, 55b), each including at least one pair of flanges (30) spaced by a transverse rail (35), and a fastener (75) for securing the pair of opposed clamping sections (55a, 55b) in clamping engagement with a barrel (20) and magazine extension (25) of a firearm. The flanges (30) are adapted to couple a barrel (30) and a magazine extension (25) to a firearm and permit attachment of accessories to the firearm.
