A rotational atherectomy device that includes an expandable head that can clean a blockage from vessel larger than its rest diameter, in which the drive shaft may rotate in two opposite directions and may have different abrasive characteristics for each rotation direction. In one direction, the head may be configured for cutting and/or slicing, which may be well suited to re-moving particularly soft blockage material. In the other direction, the head may be configured for grinding, scraping and/or sand-ing, which may be well suited to removing particularly hard blockage material. The head includes one or more arms that are piv-otally or hingedly attached to the drive shaft. One or more abrasive elements are disposed on or are attached to the one or more arms. The abrasive elements have a cutting feature, such as a sharpened edge that cuts like a razor blade when the drive shaft is ro-tated in the "cutting" direction. The abrasive elements also have a grinding feature, such as an abrasive material disposed on the abrasive element adjacent to the sharpened edge, which grinds away a blockage when the drive shaft is rotated in the "grinding" direction.
L'invention concerne un dispositif rotatif d'athérectomie qui présente une tête dilatable qui peut éliminer dans un vaisseau un colmatage plus grand que son diamètre au repos et dont la tige d'entraînement peut tourner dans deux sens opposés et peut présenter différentes caractéristiques d'abrasion dans chaque sens de rotation. Dans un sens de rotation, la tête peut être configurée pour découper et/ou former des tranches, ce qui peut bien convenir pour enlever un colmatage en matériau particulièrement mou. Dans l'autre sens de rotation, la tête peut être configurée pour meuler, gratter et/ou sabler, et peut bien convenir pour enlever un colmatage en matériau particulièrement dur. La tête comprend un ou plusieurs bras qui sont reliés par pivotement ou de manière articulée à la tige d'entraînement. Un ou plusieurs éléments abrasifs sont disposés sur le ou les bras ou y sont fixés. Les éléments abrasifs présentent un élément de coupe, par exemple une arête tranchante qui coupe à la manière d'une lame de rasoir lorsque la tige d'entraînement tourne dans le sens de la "découpe". Les éléments abrasifs présentent également un élément de meulage, par exemple un matériau abrasif disposé sur l'élément abrasif en position adjacente à l'arête tranchante, et qui enlève un colmatage par meulage lorsque la tige d'entraînement est tournée dans le sens du "meulage".
