A method for treating an osteochondral defect or a chondral defect in a subject includes implanting a composite in a site of the osteochondral or chondral defect. The composite includes a hyaluronic acid derivative; and at least one member of the group consisting of a cell, a cellular growth factor and a cellular differentiation factor, which is impregnated in, or coupled to, the hyaluronic acid derivative. In one embodiment, carboxyl functionalities of the hyaluronic acid derivative are. each independently derivatized to include an N-acylurea or O-acyl isourea, or both N-acylurea and O-acyl isourea. In another embodiment, the hyaluronic acid derivative is prepared by reacting an uncrosslinked hyaluronic acid with a biscarbodimide in the presence of a pH buffer in a range of between about (4) and about (8). The composite can be used for regenerating or stimulating regeneration of meniscal tissues in a subject in need thereof.
L'invention concerne un procédé destiné à traiter un défaut chondral ou ostéochondral chez un sujet par implantation d'un composite dans un site du défaut chondral ou ostéochondral. Le composite contient un dérivé d'acide hyaluronique; et au moins un élément du groupe constitué d'une cellule, d'un facteur de croissance cellulaire et d'un facteur de différenciation cellulaire, qui est imprégné dans, ou couplé au dérivé d'acide hyaluronique. Dans l'un des modes de réalisation, des fonctionnalités carboxyles du dérivé d'acide hyaluronique sont chacune dérivées de façon indépendante de manière à inclure du N-acylurée ou de l'O-acyl-isourée, ou les deux. Dans un autre mode de réalisation, le dérivé d'acide hyaluronique est préparé par mise en réaction d'un acide hyaluronique non réticulé avec un biscarbodiimide en présence d'un tampon de pH compris entre environ 4 et 8. Le composite peut être utilisé en vue de régénérer ou de stimuler la régénération des tissus ménisquaux chez un sujet le nécessitant.
