The present invention provides a method for bone marrow transplantation from an HLA-nommatched donor to a patient which comprises conditioning the patient under a suitable regimen followed by transplant of a very large dose of stem cells which is at least about 3-fold greater than the conventional doses used in T cell-depleted bone marrow transplantation. The patient is conditioned under lethal or supralethal conditions for the treatment of malignamt or non-malignant diseases, or under sublethal conditions for the treatment of non-malignant diseases. The transplant may consist of T cell-depleted bone marrow stem cells and T cell-depleted stem cell-enriched peripheral blood cells from the HLA-nommatched donor, preferably a relative of the patient, which donor was previously treated with a drug, e.g. a cytokine such as granulocyte colony-stimulating factor (G-CSF).

L'invention porte sur un procédé de transplantation de moelle entre un donneur n'ayant pas le même HLA (groupe leucocytaire) que le patient, et consistant à conditionner le patient à l'aide d'un régime adéquat puis à lui transplanter une importante dose de cellules souches au moins trois fois plus importante que la dose usuelle de moelle osseuse à faible teneur en lymphocytes T. Le patient est placé dans des conditions létales ou supralétales pour le traitement d'affections malignes ou non malignes, ou dans des conditions sublétales pour le traitement d'affections non malignes. Le transplant peut consister en des cellules souches de moelle osseuse appauvrie en lymphocytes T et en cellules de sang périphérique enrichies en cellules souches appauvries en lymphocytes T, provenant d'un donneur n'ayant pas le même HLA, de préférence d'un parent du patient, préalablement traité lui-même au moyen d'un médicament, par exemple une cytokine telle que le facteur stimulant de la prolifération de granulocytes (G-CSF).
