An implantable automatic cardioverter/defribrillator device for a cardiac patient is automatically responsive to sensing of electrical cardiac activity of the heart of a patient in which the device is implanted for detection and treatment of fibrillation. A prescribed electrical shock waveform regimen is generated as electrical defibrillation therapy for application to the heart in response to detection of fibrillation of the heart. A timing function circuit responds to the detection by timing the delivery of the generated prescribed electrical waveform regimen to be applied at a point in time at which the fibrillation ECG has substantially its highest amplitude and lowest frequency, as a point of high susceptibility to reversion to sinus rhythm upon application of a shock, and with low defibrillation threshold, to enhance the probability of successful defibrillation, and to synchronize the shock delivery to this sensed intracardiac event.

L'invention concerne un défibrillateur automatique implantable pour des patients atteints de troubles cardiaques permettant de surveiller automatiquement l'activité électrique cardiaque et de réagir en cas de détection d'une fibrillation, pour la traiter en administrant un régime prédéterminé d'ondes électriques constituant un électrochoc. Cette thérapie de défibrillation électrique du coeur est appliquée lorsqu'une fibrillation du coeur est détectée. Un circuit de programmation de l'électrochoc réagit à cette détection en programmant l'administration du régime prédéterminé d'ondes electriques pour qu'elle ait lieu à l'instant où la fibrillation selon l'ECG a l'amplitude la plus élevée et la fréquence la plus basse, ce qui représente l'instant de susceptibilité optimum de retour à un rythme sinusoïdal, sur application de l'électrochoc selon un seuil de défibrillation peu élevé, ce qui permet de synchroniser l'administration de l'électrochoc avec l'incident intracardiaque détecté et d'accroître ainsi les chances de succès.
