A method of diagnosis and reduction of dental caries is provided. The method comprises testing for Streptococcus mutans in an oral cavity of a patient and determining the patient's risk level for developing dental caries, to establish what treatment, if any, the patient requires in accordance with the following guidelines: a. objectively measuring the level of Streptococcus mutans in the patient's oral cavity b. for a patient with above normal levels of Streptococcus mutans in the oral cavity, the patient is given a series of treatments with at least one antimicrobial agent, in combination with a series of fluoride treatments, over a period of about 1.5 years; c. for a patient with substantially greater than normal or persistently above normal levels of Streptococcus mutans, the patient is given a series of treatments with at least one antimicrobial agent, in combination with a series of fluoride treatments over of about 2 years; d. for a patient with slightly above normal levels of Streptococcus mutans, the patient is given a series of fluoride treatments between recall visits to the dentists; e. patients who fall within the parameters of steps b. and c., above, are re- evaluated for disease prior to the expiration of the 1.5 year or 2 year treatment period, respectively.
