Apparatus and methods for drilling out from a casing string in a borehole. A reaming assembly (10) is introduced into the borehole and fixed at a predetermined location. A drillable collar (7) is interposed between the reaming assembly (10) and a casing (3). A drilling assembly introduced into the casing (3) and is then positioned so as to drill through the drillable collar (7) at a predetermined angle into a formation (15) surrounding the borehole.
