The present invention provides catheter apparatus (10, 200) and catheterization methodology for generating an arteriovenous fistula or a veno- venous fistula on demand between closely associated blood vessels and at a chosen anatomic site in vivo. The catheter apparatus is preferably employed in pairs, each catheter of the pair being suitable for percutaneous introduction into and extension through a blood vessel. The catheterization methodology employs the catheter apparatus preferably in conjunction with conventional radiological techniques in order to place, verify, and confirm a proper alignment, orientation, and positioning for the catheters in vivo prior to activating the perforation means for generating a fistula. The invention permits the generation of arteriovenous fistulae and veno-venous fistulae anatomically anywhere in the vascular system of a patient; nevertheless, the invention is most desirably employed in the peripheral vascular system as exists in the extremities of the body to aid in the treatment of the patient under a variety of different medical ailments and pathologies.
L'invention concerne un cathéter (10, 200) et une méthodologie de cathétérisme permettant de générer une fistule artério-veineuse ou veino-veineuse sur demande entre des vaisseaux sanguins proches et au niveau d'un site anatomique présélectionné in vivo. Le cathéter est de préférence employé par paires, chaque cathéter de la paire pouvant être introduit en percutané dans un vaisseau sanguin et y être avancé. La méthodologie emploie le cathéter de préférence en association avec les techniques de radiologie conventionnelles afin de mettre en place les cathéters, et de vérifier et de confirmer que l'alignement, l'orientation et le positionnement des cathéters in vivo sont corrects, avant d'activer le système de perforation pour créer une fistule. L'invention permet de créer des fistules artério-veineuses et veino-veineuses n'importe où, du point de vue anatomique, dans le système vasculaire d'un patient; néanmoins, elle s'emploie de préférence dans le système vasculaire périphérique tel qu'il existe au niveau des extrémités du corps, afin de contribuer à traiter un patient pour une variété de pathologies.
