A first catheter surrounds a guidewire, with the first catheter and guidewire each having a distal portion emplaced in the cardiovascular system of a patient. By this invention, the first catheter may be removed without displacing the guidewire. To accomplish this, one may advance an exchange catheter into the patient to a desired position where the exchange catheter completely surrounds the guidewire distal end and, in turn, is surrounded along part of its length by the first catheter. One causes a proximal end portion of the guidewire to project outwardly through a longitudinal slit of the exchange catheter, so that the proximal end of the guidewire may be grasped. By this means the guidewire may be retained in the desired position as the exchange catheter is advanced into its position within the first catheter and surrounding the guidewire. One then withdraws the first catheter from the patient, while causing the exchange catheter to be held relatively stationary. Thus the guidewire within the exchange catheter is also retained in relatively stationary position without the need for retention of the guidewire by manual grasping. Also, the guidewire may be replaced through the exchange catheter.
Cette invention concerne une méthode de remplacement de cathéter. Un premier cathéter entoure un fil-guide, les deux comportant une extrémité distale implantée dans l'appareil cardio-vasculaire d'un patient. Selon la présente invention, le premier cathéter peut être retiré sans déplacer le fil-guide. Pour ce faire, le praticien fait pénétrer un cathéter de remplacement dans le corps du patient jusqu'à la position voulue, soit à un endroit où ce cathéter recouvre complètement l'extrémité distale du fil-guide et où il est lui-même recouvert en partie par le premier cathéter. Par la suite, le praticien sort un tronçon proximal du fil-guide du cathéter de remplacement par une fente longitudinale y pratiquée de façon à pouvoir le saisir. Cette procédure permet de retenir le fil-guide à l'endroit voulu pendant que le cathéter de remplacement est poussé à sa place à l'intérieur du premier cathéter et autour du fil-guide. Ensuite, le praticien retire le premier cathéter du corps du patient tout en maintenant à peu près immobile le cathéter de remplacement. Cette procédure permet de maintenir également le fil-guide à peu près immobile à l'intérieur du cathéter de remplacement et ce, sans avoir à le manipuler. De plus, le fil-guide peut être remplacé via le cathéter de remplacement.
