ABSTRACT A process is provided for preparing a 7-substituted- 3-cephem-4-carboxylic acid derivative of the formula: Image (VI) wherein R1 is amino or protected amino, R2 is hydrogen, halogen or hydroxy, R3 is carboxy or functionally modified carboxy, and A2 is lower alkylene substituted with a group of the formula : ?-O-R4 in which R4 is hydrogen, lower alkyl, lower alkenyl, lower alkynyl or cycloalkyl, and its nontoxic, pharmaceutically acceptable salts, which comprises reacting a compound of the formula. Image (Id') wherein R1, R2 and R3 are each as defined above, and A4 is lower alkylene substituted with oxo, or its salt, with a compound of the formula: R4-ONH2 wherein R4 is as defined above, the derivatives have antimicrobial activity against Gram negative and Gram positive bacteria.
