The invention relates to cobalamin derivatives (a) having no binding affinity or low binding affinity to the transport protein transcobalamin II TCII) and (b) retaining activity as a vitamin B12 substitute, optionally carrying a therapeutic and/or diagnostic agent, such as a radioactive metal. These compounds have a much reduced accumulation rate in blood and benign organs, such as kidney and liver, compared to the accumulation rate in neoplastic tissues, and are more rapidly eliminated from blood. The invention further relates to a method of diagnosis and a method of treatment of a neoplastic disease or an infection by microorganisms in a mammal comprising (a) exposing the mammal to a period of a vitamin B12 - free diet, and (b) subsequently applying a cobalamin derivative of the invention carrying a diagnostic and/or therapeutic agent. By selecting cobalamin derivatives acting as vitamin B12 substitutes, the risk of the formation of resistant off~spring in neoplastic tissue is much reduced.

L'invention porte sur des dérivés de la cobalamine: (a) présentant peu ou pas d'affinité de liaison avec la protéine de transport transcobalamine II (TCII), et (b) présentant l'activité d'un substitut de la vitamine B12 et contenant facultativement un agent thérapeutique et/ou diagnostique tel qu'un métal radioactif. Ces composés présentent un taux réduit d'accumulation dans le sang et les organes bénins tels que les reins et le foie, par comparaison avec le taux d'accumulation dans les tissus néoplasiques, et s'éliminent plus rapidement du sang. L'invention porte également sur une méthode de diagnostique et sur une méthode de traitement de maladies néoplasiques ou d'infections dues à des micro-organismes, de mammifères consistant (a) à leur faire subir un régime sans vitamine B12 pendant une période, et (b) à leur appliquer un dérivé de la cobalamine comprenant un agent diagnostique ou thérapeutique. En choisissant ces dérivés de la cobalamine comme substituts de la vitamine B12, on réduit fortement le risque de formation de surjets de tissus néoplasiques.
