IMPROVED COMPOSITE JOINTED FORM ABSTRACT A composite jointed form defined by a first strip of printable sheet-like material having opposed straight parallel side edges. A composite strip of sheet-like material is secured along a first of the side edges of the first strip. The composite strip is comprised of a backing sheet, having an adhesive release material on a top side thereof, and opposed parallel side edges with one of the side edges disposed closely spaced along the first side edge of the first strip. A frontal sheet of printable sheet-like material, having a pressure sensitive adhesive on the back side thereof, is disposed in facial contact with the adhesive release material of the backing sheet. The frontal sheet has a connecting edge portion which extends beyond the first side edge over a top edge portion of the first strip. The frontal sheet has an information carrying portion which is detachable from the backing sheet.
