Compositions and methods are provided for treatment of cartilage defects in animals and humans. The compositions of the invention include synovial tissue, synovial cells and matrices containing synovial (or cambium) tissue or cells for use in filling a cartilage defect. The matrix and synovial tissue or cell preparations may also contain a proliferation agent, transforming factor or other active agents to promote healing. A controlled- release delivery system may be used to administer the transforming factor. The compositions of the invention also include a synovial covering membrane or devitalized fascial sheet for covering the cartilage defect. The methods of this invention are those in which a minimally invasive surgical intervention is performed to remove a small portion of synovial membrane from a joint. Portions of the synovial membrane, or cells expanded in vitro, are implanted alone or within a matrix, into the defect site, where they produce new cartilage tissue and repair the defect. Alternatively, partially transformed synovial-derived tissue may be formed in situ and implanted into the defect site.
L'invention concerne des compositions et des méthodes permettant de traiter des défauts de cartilage chez des animaux et des humains. Les compositions de cette invention contiennent du tissu synovial, des cellules synoviales et des matrices renfermant des cellules ou du tissu synovial (ou cambium) destinés au remplissage d'un défaut du cartilage. Les préparations cellulaires ou tissulaires synoviales et matricielles peuvent également renfermer un agent de prolifération, un facteur de transformation ou d'autres agents actifs de manière à favoriser la guérison. On peut utiliser un système d'administration à libération régulée pour administrer le facteur de transformation. Lesdites compositions de l'invention comprennent aussi une membrane de revêtement synovial ou une feuille des fascias dévitalisée permettant de recouvrir le défaut de cartilage. Les méthodes de cette invention sont celles avec lesquelles on réalise une intervention chirurgicale très peu effractive destinée à enlever une petite portion de la membrane synoviale à partir d'une articulation. On implante des portions de la membrane synoviale ou des cellules expansées <i>in vitro</i>, seules ou à l'intérieur d'un matrice, dans le site de défaut, où ces portions produisent du nouveau tissu cartilagineux et réparent le défaut. Selon un autre mode de réalisation, on peut former <i>in situ</i> un tissu dérivé du liquide synovial partiellement transformé et l'implanter dans le site de défaut.
