A multi-layer, multi-web, laser printable, page-sized form (20) with an identification band blank (28) comprises a multi-layered web die cut with a first paper label portion (30) for receiving a printed image and .an adhesive backed transparent film layer (36) approximately twice the width of the paper layer so that upon separation from a carrier, the transparent film may be folded over and completely encapsulate the paper layer to moisture proof it and protect it. The transparent film layer (36) includes a pair of adhesive backed tabs (44, 46) at the ends to facilitate its attachment about a patient's wrist or ankle. The band blank web (60) is joined to a second multi-layer web (64) with an overlapping glued joint, the second mufti-layer web having a plurality of adhesive backed labels (74) die cut into it.
L'invention se rapporte à un formulaire (20) ayant la taille d'une page, imprimable au laser, à feuilles multiples et multicouche, qui comporte une ébauche (28) de bande d'identification et est composé d'une feuille multicouche coupée à la forme et comprenant une première partie étiquette (30) en papier destinée à recevoir une image imprimée et une couche (30) de film transparent et adhésif ayant approximativement deux fois la largeur de la couche en papier de sorte que lors de la séparation d'avec un support, le film transparent peut être replié et envelopper complètement la couche en papier de façon à la rendre résistante à l'humidité et à la protéger. La couche (36) de film transparent comporte une paire de languettes (44, 46) adhésives au niveau de ses extrémités dans le but de faciliter sa fixation autour du poignet ou de la cheville d'un patient. La feuille (60) d'ébauche de bande est assemblée à une seconde feuille (64) multicouche par assemblage collé à recouvrement, une pluralité d'étiquettes (74) adhésives étant coupées à la forme dans la seconde feuille multicouche.
