The present invention discloses an identification band blank, cut into and removable from a multi-layered form suitable for processing through a computer controlled printer. The multi-layered blank has a first layer suitable for receiving a print image from the printer, a second, over-sized layer of moisture resistant material, and a pair of integrally formed adhesive-backed tabs cut into and formed out of the material comprising the second layer and near the ends of the band blank so that upon removal of the band blank from the form the second layer may be folded over to overlie the printed image on the first layer and the tabs used to secure the band blank about an appendage of a person.
