1 The invention provides a simplified method of preparing a hole in a poured concrete form for the insertion of a expandable anchoring device, or core hole once the forms are removed. The cylinder body has a hollow center which accepts a hex head screw to allow attachment to a wood form. The diameter of the base of the cylinder body is larger than the diameter of the opposite end of the cylinder body to allow easy removal of the form when the wood forms are removed. The device may be attached to any surface where once the concrete is hardened that surface will be removed. As the size of the hole required increases so does the size of the cylinder body. As the size of the cylinder body increases to become a core hole rather than a hole for a expandable anchoring device the base and opposite end of the cylinder body breakaway as the surface it is attached to is removed leaving a liner for the hole.
