A method and devices for localized delivery of a chemotherapeutic agent to solid tumors, wherein the agent does not cross the blood--brain barrier and is characterized by poor bioavailability and/or short half-lives in vivo, are described. The devices consist of reservoirs which release drug over an extended time period while at the same time preserving the bioactivity and bioavailability of the agent. In the most preferred embodiment, the device consists of biodegradable polymeric matrixes, although reservoirs can also be formulated from non--biodegradable polymers or reservoirs connected to implanted infusion pumps. The devices are implanted within or immediately adjacent the tumors to be treated or the site where they have been surgically removed. The examples demonstrate the efficacy of paclitaxel, camptothecin, and carboplatin delivered in polymeric implants prepared by compression molding of biodegradable and non-biodegradable polymers, respectively. The results are highly statistically significant.
Procédé et dispositifs permettant d'apporter de manière localisée un agent chimiothérapeutique à des tumeurs solides, cet agent ne traversant pas la barrière hématoencéphalique et se caractérisant par une faible biodisponibilité et/ou des demi-vies de courte durée in vivo. Les dispositifs sont constitués de réservoirs qui libèrent un médicament pendant une période prolongée tout en présentant la bioactivité et la biodisponibilité de l'agent. Dans la forme de réalisation préférée le dispositif est constitué de matrices polymères biodégradables bien que les réservoirs puissent également être formulés à partir de polymères non biodégradables ou de réservoirs connectés à des pompes à perfusion implantées. Les dispositifs sont implantés dans les tumeurs ou immédiatement à côté des tumeurs à traiter ou du site où elles se trouvaient avant leur élimination par chirurgie. Les exemples démontrent l'efficacité du paclitaxel, de la camptothécine et du carboplatine libérés dans des implants polymères préparés par moulage par compression de polymères biodégradables et non biodégradables, respectivement. Les résultats sont profondément significatifs du point de vue statistique.
