Atrial fibrillation (AF) is frequently encountered in patients with heart failure (HF) and is also a predictor of morbidity and mortality in this population. Recent experimental studies have shown electrical and structural atrial remodeling with increased fibrosis in HF animals, and have suggested a preventive effect of angiotensin converting enzyme inhibitors (ACEi) on the development of AF. To verify the hypothesis that ACEi prevent the development of AF in patients with HF, a retrospective analysis of the patients from the Montreal Heart Institute (MHI) included in the Studies Of Left Ventricular Dysfunction (SOLVD) was conducted. The results of this retroactive analysis indicate that treatment with the ACE inhibitor, such as enalapril, can markedly reduce the risk of developing atrial fibrillation in patients with left ventricular dysfunction.

La fibrillation auriculaire (FA) se rencontre fréquemment chez des patients atteints d'insuffisance cardiaque (IC) et constitue également un prédicteur de morbidité et de mortalité au sein de cette population. Des études expérimentales récentes ont montré un remodelage auriculaire électrique et structural avec augmentation de la fibrose chez des animaux atteints de IC et ont révélé l'effet préventif des inhibiteurs de l'enzyme de conversion de l'angiotensine (iECA) sur l'évolution de la FA. Pour vérifier l'hypothèse selon laquelle les iECA préviennent l'apparition de la FA chez des patients atteints de IC, une analyse rétrospective des patients du Montreal Heart Institute (MHI) incluse dans les Studies Of Left Ventricular Dysfunction (SOLVD) (Etudes de la dysfonction ventriculaire gauche) a été menée. Les résultats de cette analyse rétroactive indiquent qu'un traitement à l'aide d'inhibiteur d'ECA, tel que l'énalapril, peut réduire de façon marquée le risque d'apparition de fibrillation auriculaire chez des patients atteints de dysfonction ventriculaire gauche.
