A system for performing an end-to-side vascular anastomosis, inclunding an anastomosis device, an application instrument and methods for performing a vascular anastomosis. The system is applicable for performing an anastomosis between a vascular graft and the ascending aorta in coronary artery bypass surgery, particularly in port-access CABG surgery. A first aspect of the invention includes a vascular anastomosis staple. A first configuration has two parts: an anchor member (101), forming the attachment with the target vessel wall and a coupling member (102) forming the attachment with the bypass graft vessel. The anastomosis is completed by inserting the coupling member (102), with the graft vessel attached, into the anchor member (101). A second configuration combines the functions of the anchor member and the coupling member into a one-piece anastomosis staple. A second aspect of the invention includes an anastomotic fitting, having an inner flange over which the graft vessel is everted and an outer flange which contacts the exterior surface of the target vessel.

L'invention concerne un système pour effectuer une anastomose vasculaire termino-latérale et plus particulièrement, un dispositif d'anastomose, un instrument d'application et des procédés pour réaliser l'anastomose. Ce système permet d'effectuer une anastomose entre un greffon veineux et l'aorte ascendante dans la chirurgie par pontage, et plus spécialement, dans le pontage coronarien par greffe veineuse. Dans un premier aspect, l'invention comprend une agrafe d'anastomose vasculaire. Une première configuration comporte deux parties: un élément d'ancrage (101), constituant la fixation à la paroi du vaisseau cible et un élément de couplage (102), formant la fixation au greffon veineux utilisé pour le pontage. L'anastomose est réalisée par insertion de l'élément de couplage (102), une fois le greffon veineux fixé, dans l'élément d'ancrage (101). Une deuxième configuration combine les fonctions de l'élément d'ancrage et de l'élément de couplage en une agrafe d'anastomose formée d'un seul tenant. Un deuxième aspect de l'invention concerne un raccord d'anastomose, présentant un rebord intérieur sur lequel le greffon veineux est placé en éversion, et un rebord extérieur qui vient en contact avec la surface extérieure du vaisseau cible.
