ABSTRACT OF TEE DISCLOSURE A mailer type business form is formed by double parallel folding a single sheet of paper. The mailer may include a return envelope with a side opening edge, or it may comprise a seven page booklet, all of the pages attached together at one edge and most or all of the pages detachable from the booklet along a perforation line. One of the panels may be of a somewhat smaller width than the others, to allow elimination of one adhesive line, and one thickness of the final mailer, when the intermediate is double parallel folded.
