An apparatus and method is disclosed to resect and remove tissue. The apparatus is particularly suited for operation on the prostate for treatment of Benign Prostatic Hyperplasia (BPH). The apparatus has a deflectable member (18),which may be a wire, and which conducts electrosurgical current. The deflectable member (18) protrudes from an opening (22) near the distal end (13) of a hollow tube. Tissue is cut and coagulated by the deflectable member (18) asthe apparatus is rotated. A sharp tip on the distal end (13) of the tube allows the apparatus to be inserted percutaneously. Suction and irrigation are integral to the apparatus. One advantage of the apparatus is that tissue samples can be recovered for pathological analysis. Other advantages are that operations on theprostate can be conducted with minimal blood loss, concerns about TUR syndrome are reduced, and the bladder neck and urethra can be spared. Use of this device on the prostate also can eliminate the need for a post-operative catheter and provides immediate post-operative relief.
Appareil et méthode pour la résection et l'ablation de tissus. L'appareil convient particulièrement à l'opération de la prostate en vue du traitement de l'hypertrophie prostatique bénigne. L'appareil possède un élément orientable (18), qui peut être un fil métallique et qui peut conduire un courant à des fins électrochirurgicales. L'élément orientable (18) fait saillie par une ouverture (22) près de l'extrémité distale (13) d'un tube creux. Le tissu est coupé et coagulé par l'élément orientable (18) lorsqu'on qu'on fait faire une rotation à l'appareil. Une pointe coupante à l'extrémité distale (13) du tube creux permet d'insérer l'appareil par voie percutanée. L'aspiration et l'irrigation font partie intégrante de l'appareil. Un avantage de l'appareil tient au fait que les prélèvements de tissu peuvent être récupérés pour examen anatomo-pathologique. Autres avantages : les opérations de la prostate peuvent être réalisées avec un minimum de pertes sanguines, les préoccupations liées au syndrome de résection transurétrale endoscopique sont réduites, et le col vésical et l'urètre sont épargnés. L'utilisation de ce dispositif pour l'opération de la prostate peut également écarter la nécessité d'utiliser un cathéter post-opératoire et assure un soulagement immédiat après l'opération.
