A portable console (16) includes a universal positioning arm assembly ( 18) for holding a surgical instrument such as an endoscope (14) during a surgical procedure. Coarse adjustment of instrument elevation above an operating table ( 12) is provided by a reversible drive motor ( 58) which extends and retracts an upright support shaft (24) along a vertical axis (A). Coarse adjustment of instrument X-Y position within the sterile zone above an operating table is provided by articulated arm sections (20, 22, 90, 100) which are rotatably coupled to the upright support shaft. Fine adjustment of instrument position across the sterile zone is provided by a revers- ible drive motor (84) which extends and retracts the instrument along the longitudinal axis (C) of the support arm section (22). Fine adjustment of instrument elevation is provided by a reversible drive motor (92) which extends and retracts the distal support arm section (100) along the axis (D). Coarse positioning is obtained by actuation of console switches (70, 72, 74) and an arm-mounted switch (76), and fine positioning is obtained by foot pressure applied to floor switches (120, 122, 124, 126, 128) by the operating surgeon.
