A fall protection insert for a manhole, includes a body adapted for insertion down a manhole. The body has a peripheral edge. Between three and five engagement members, are mounted along the peripheral edge of the body. Each of the engagement members are movable between a retracted insertion position and an extended engagement position. In the extended engagement position each of the engagement members extend outwardly past the peripheral edge of the body, thereby engaging the peripheral walls of a manhole. A lever is provided for selectively moving each of the engagement members from the retracted insertion position to the extended engagement position.
