The form is comprised of foamed-plastic panels held parallel to each other and properly spaced by connecting members, the ends of which are embedded in the foamed-plastic panels. The ends of the connecting members which are located at the bend in the form are properly positioned and of suitable shape and material for receiving fasteners.
