Based upon patient studies, a high degree of correlation was found between HRV, and VO2max, i.e., the patient's exercise capacity. A pacing therapy optimization protocol for treating patients with CHF has been devised. An HRV index is derived during a first pacing mode, then the mode is changed, and the steps repeated for all modes. The pacemaker mode with the highest HRV index is then programmed. Alternatively the method can apply to changes in drug therapy instead, or in combination with pacing therapy.
En fonction d'indices provenant d'études sur des patients, on a trouvé un haut degré de corrélation entre le HRV (variabilité du rythme cardiaque) et le VO2¿max? (fixation maximale d'oxygène) c.-à-d. la capacité d'exercice du patient. On en a conçu un protocole de traitement des patients souffrant de défaillances cardiaques, puis on en a tiré un indice HVR dans un premier mode de stimulation. On modifie ensuite le mode et répète les étapes pour tous les modes. Le mode présentant l'indice HVR le plus élevé est alors programmé. En variante le procédé peut s'appliquer à des modifications de thérapies médicamenteuses ou, à leur place ou en association, à une thérapie par stimulation.
