Methods and substances for determining the presence of Islet Cell Antibodies (ICA) in a sample are provided. The methods include contacting the sample with a pancreatic lipid preparation and detecting the presence of an immunoreaction between the ICA and the pancreatic lipid fraction. The novel pancreatic lipid fraction contains a lysophosphatidylinositol. The methods and substances are useful for diagnosing patients suspected of having diabetes or at risk for the onset of diabetes in the future.
