A method and apparatus for stimulating the right and/or left carotid sinus nerves or the right stellate ganglion or the epidural space at about T2 with continuous and/or phasic electrical pulses in response to detected myocardial ischemia to decrease cardiac workload as a method to protect the myocardium. The automatic detection of the need for such stimulation is responsive to a change in ST segment variation different from a predetermined or programmed threshold suggesting acute myocardial ischemia and/or other criteria or indicators of myocardial ischemia, including changes if pH and/or oxygen saturation (SO2) detected from a sensor located in the heart, preferably the coronary sinus. The system is implemented with a dual chamber pacemaker with programming and telemetry capabilities for automatically recording in memory the aforementioned values for periods of time preceding and following each instance of stimulation for telemetry out on command and for programming "on" one or more of the sensors.
Méthode et appareil pour stimuler les nerfs des sinus carotidiens gauche et droit, ou l'un des deux, ou le ganglion étoilé droit ou l'espace épidural environ au niveau de la D2 avec des impulsions électriques continues et/ou rythmées en réponse à la détection d'une ischémie du myocarde afin de réduire le travail du coeur et ainsi protéger le myocarde. La détection automatique du besoin d'une telle stimulation fait suite à un changement dans la variation du segment ST différent d'un seuil prédéterminé ou programmé qui suggère une ischémie aiguë du myocarde et/ou à d'autres critères ou indicateurs d'une ischémie du myocarde, y compris des changements dans le pH et/ou dans la saturation en oxygène (SO2) détectés par un capteur dans le coeur, de préférence dans le sinus coronaire. Le système est appliqué avec un stimulateur cardiaque à deux chambres avec capacités de programmation et de télémétrie pour la mémorisation automatique des valeurs susmentionnées pendant des périodes précédant et suivant chaque stimulation pour la transmission d'une télémesure sur commande et pour « activer » un ou plusieurs capteurs.
