A method and system for treatment of urinary incontinence includes the injection or laparoscopic implantation of one or more battery- or radiofrequency-powered microstimulators (10) beneath the skin of the perineum. The devices are programmed using radio-frequency control via an external controller (20, 30) that can be used by a physician to produce patterns of output stimulation pulses judged to be efficacious by appropriate clinical testing to diminish incontinence symptoms. The stimulation program is retained in the microstimulator device (10) or external controller (20) and is transmitted when commanded to start and stop by a signal from the patient or caregiver. The system and method reduce the incidence of unintentional episodes of bladder emptying by stimulating nerve pathways (8) that diminish involuntary bladder contractions, improve closure of the bladder outlet, and/or improve the long-term health of the urinary system by increasing bladder capacity and emptying. Further, the system and method allow a patient to be taught to receive one or more patterns of neural stimulation that can be prescribed by a physician and administered without continuous oversight by a clinical practitioner.
L'invention concerne un procédé et un système de traitement de l'incontinence urinaire, le procédé consistant à injecter ou à implanter par laparoscopie un ou plusieurs microstimulateurs (10) à alimentation radiofréquence ou par batterie, sous la peau du périnée. Les dispositifs sont programmés par commande radiofréquence via une commande (20, 30) extérieure qui peut être utilisée par un médecin pour produire des formes d'impulsions de stimulation de sortie jugées efficaces, suite à des essais cliniques appropriés, pour diminuer les symptômes d'incontinence. Le programme de stimulation est gardé dans le microstimulateur (10) ou la commande (20) extérieure et est transmis lorsqu'un signal envoyé par le patient ou le soignant en commande la marche ou l'arrêt. Le système et le procédé selon l'invention permettent de réduire l'incidence d'épisodes accidentels d'évacuation de la vessie par une stimulation des voies (8) nerveuses qui diminue les contractions involontaires de la vessie, améliore l'obturation de l'orifice de la vessie et/ou améliore la santé à long terme du système urinaire en augmentant la capacité de la vessie et son évacuation. En outre, les système et procédé permettent d'apprendre à un patient à recevoir une ou plusieurs formes de stimulation neuronale pouvant être prescrites par un médecin et administrées sans supervision continue par un praticien clinique.
