An inflatable rectal insert applies both pressure and cold therapy to rectal tissues. A plastic housing defines a relatively large support and a slender stem extending from the support and dimensioned for insertion into the rectum. An inflatable jacket surrounds the stem, and a compartment within the support contains a charge of liquid coolant used to inflate the jacket. An opening in the exterior of the housing places the compartment in communication with the interior of the jacket for liquid transfer. A cap attached to the support is rotated to displace a piston within the housing. Displacement of the piston pumps the liquid coolant between the compartment and the interior of the jacket. -15-
