An information accessory device particularity adaptable for use with telecommunication devices, such as telephones, computers, fax machines, and the like, consisting of two parts, i.e., a housing or slide guide and a slide means, the latter containing printed information thereon. The parts are constructed of relatively thin and rigid sheet plastic and are die cut from the sheet material. The housing or slide guide is heated and opposing sides are deformed inwardly toward one another to provide opposing channels to receive the slide means, which has a width substantially the same as the space between the channels and a length much in excess of the housing. Lugs or tabs are formed at one terminal end of the slide means to engage the ends of the channels to prevent removal of the slide means when moved in one direction. The other terminal end of the slide means is creased and bent to form a handle-like portion for grasping and to prevent removal of the slide means from the channels when the slide is moved in the opposite direction. The slide means may be of multiple members, one above the other, in ascending lengths from top to bottom, with each member being of the same width as the others.
