A method of treatment of disease by inhibition of cellular secretory processes is provided. The method has particular application in the treatment of diseases dependent upon the exocytotic activity of endocrine cells, exocrine cells, inflammatory cells, cells of the immune system, cells of the cardiovascular system, and bone cells. Agents and compositions therefor, as well as methods for manufacturing these agents and compositions, are provided. In a preferred embodiment a clostridial neurotoxin, substantially devoid of holotoxin binding affinity for neuronal cells of the presynaptic muscular junction, is associated with a targeting moiety. The targeting moiety is selected such that the clostridial toxin conjugate so formed may be directed to a non-neuronal target cell to which the conjugate may bind. Following binding, a neurotoxin component of the conjugate, which is capable of inhibition of cellular secretion, passes into the cytosol of the target cell by cellular internalisation mechanisms. Thereafter, inhibition of secretion from the target cell is effected.

L'invention concerne une méthode de traitement d'une maladie par l'inhibition de processus de sécrétion cellulaire. La méthode trouve une application particulière dans le traitement de maladies qui dépendent de l'activité exocytotique de cellules endocrines, exocrines, inflammatoires, cellules du système immunitaire, du système cardiovasculaire et des cellules osseuses. L'invention concerne également des agents et des compositions à cet effet, ainsi que des procédés de préparation de ces agents et compositions. Dans un mode de réalisation, une neurotoxine clostridiale, sensiblement dépourvue d'affinité de liaison à l'holotoxine pour des cellules neuronales de la jonction musculaire présynaptique, est associée à une fraction de ciblage. Cette dernière est sélectionnée de façon que le conjugué de toxine clostridiale ainsi formé soit dirigé vers une cellule cible non neuronale à laquelle le conjugué puisse se lier. Après la liaison, un composant de neurotoxine du conjugué, capable d'inhiber la sécrétion cellulaire, pénètre dans le cytosol de la cellule cible par des mécanismes d'internalisation cellulaire. L'inhibition de la secrétion de la cellule cible est ensuite effectuée.
