Insertion and extraction tool, systems and methods for use with lacrimal implants. An insertion tool is disclosed that includes a tool body (200) having a distal portion configured to hold an implant on an outer implant surface, the distal portion having in inner lumen with an internal depth stop (425), and a plunger slidable (430) within the inner lumen to engage and dispense the implant, the plunger having a stop configured to engage with the internal depth stop, wherein the engagement of the stop and internal depth stop limits the depth of insertion of the implant into the punctum.
