ABSTRACT OF THE DISCLOSURE An instrument for surgically fastening tissue using a pronged fastener engageable in a retainer has an elongate rigid piercing member slidable through channels in the fastener prongs for piercing openings for the prongs in the tissue. Cam operated mechanisms are provided for firstly displacing the piercing member through the tissue to form the openings and then urging the prongs through the openings into engagement with retainer holes in the retainer.
